Project: ICT Integrated System for Coordinated Polypharmacy Management in Elders with Dementia

Project Topic | MedGUIDE will feature an integrated set of ICT services offering personalized support for coordinated dementia care and self-management of drug related problems associated with polypharmacy in elders which impacts their wellbeing and quality of life. The primary target group are elders with early stages of dementia while the caregivers and medical professionals/ pharmacists are secondary and tertiary target groups. The project addresses the elders’ difficulties for adhering to the medication regime impacting their wellbeing, the difficulties of medical professionals in assessing the medication side effects in their case and of pharmacists in monitoring, evaluating and reviewing their medication usage. We propose an AALsolution that offers: (i) support for improving the care and medication adherence through direct reminders and personalized roadmaps leveraging on the network of informal caregivers in helping the frail patients, (ii) key insights in the actual needs of elders with dementia by combining inputs from the patient, the network of informal caregivers and relevant contextual data from IoT devices and (iii) highlights actual medication usage and side effects in case of dementia. The employed technology will offer social-network communication and information sharing, innovative IoT monitoring, cloud data integration and big data analytics for polypharmacy self-management. The business case exploits the MedGUIDE unique selling features and builds on revenue streams identified for each target group featuring market push and pull strategies for commercializing the outcomes in 18 months after project ends while leveraging on business partners already setup network of customers. MedGUIDE will adopt a user-centred development methodology involving in the project three end-users organizations acting also as launching customers that will recruit eligible participants for developing and evaluating the proposed solutions, conforming to the ethical procedures defined. |
